When you create a flow, you are prompted for the conditions under which that flow will be triggered. It could be when a record is added or updated, called from an external program, or based on a scheduler.  The trigger can further be specified by including consitions. e.g. this flow will be triggered when a record is added, and the new record City field = “Chicago”.
